Cellulite Treatment: What Works and What Does Not

Cellulite is one of the most treated and least honestly discussed concerns in aesthetics. Before anything else, two facts worth sitting with.

Start here

Cellulite affects the large majority of women, at every size and every level of fitness. It is not a sign of being overweight, unfit or neglectful — lean athletes have it.

It cannot be cured. It can be meaningfully improved, and the improvement needs maintaining.

Any clinic promising to eliminate it is overselling, and any product promising it in a tube is not reaching the layer where the problem sits.

What cellulite actually is

Beneath the skin, fibrous bands run from the deeper tissue up to the underside of the skin, like tethers. Between and around them sits a layer of fat.

When those bands tighten or thicken and the fat between them pushes upward, the skin is pulled down in some places and pushed up in others. That is the dimpling. It is a structural issue involving three things at once — the bands, the fat and the skin above them.

This explains almost everything people find confusing about it. It explains why losing weight does not necessarily remove it, because the bands remain. It explains why creams do not work, because the problem is not at the surface. And it explains why the treatments that do help are the ones that work at depth.

Why it is more common in women

  • The fibrous bands run in a different pattern in women — more vertical, which allows fat to push between them more easily. In men they run in a criss-cross pattern that holds the tissue flatter
  • Skin is generally thinner, so what sits beneath shows more readily
  • Oestrogen influences both fat distribution and connective tissue
  • Genetics play a substantial role, which is why two people with identical habits can have very different results

It typically becomes more noticeable after puberty, during pregnancy and around menopause — all points of hormonal change rather than points of carelessness.

What does not work

  • Creams, oils and scrubs. They may temporarily firm or plump the surface, which can make skin look smoother for a few hours. They do not reach the fibrous bands, and nothing applied topically does.
  • Weight loss alone. It can help, and for some people it helps noticeably. But losing weight does not release the bands, and some people find the dimpling more visible afterwards because there is less volume filling the area.
  • Dry brushing and coffee scrubs. Pleasant, briefly stimulating, and not doing anything structural.
  • Any single session of anything. Cellulite treatment is a course with maintenance. A one-off produces a one-off result.

What genuinely helps

Radiofrequency

Radiofrequency heats the deeper layers of skin, which prompts collagen production and improves skin firmness. Firmer skin above the dimpling makes the dimpling less visible. It works on the skin layer of the problem rather than the bands.

Ultrasound cavitation

Cavitation uses ultrasound to target the fat layer. Reducing the volume of fat pushing up between the bands reduces the contrast between the raised and tethered areas. It is most useful where there is a genuine fat component rather than purely a skin-laxity one.

Lymphatic drainage and massage techniques

Improving circulation and fluid drainage reduces the puffiness that worsens the appearance. It is supportive rather than structural, and it works best alongside something else rather than alone.

Microneedling and radiofrequency microneedling

Working at depth to build collagen in the skin itself. Particularly useful where skin laxity is a large part of what you are seeing.

Fat dissolving injections

Small injections into the fat layer break down fat cells in the area, which the body then clears over the following weeks. Like cavitation, they work on the fat component rather than the bands, and they are most useful where fat is a large part of what you are seeing.

Combination, in practice

In practice the best results come from combining approaches across a course, because cellulite has three components and no single treatment addresses all three. What that combination looks like depends on which component dominates in your case — which is what the consultation is for.

What results actually look like

  • Smoother skin texture and less visible dimpling, rather than flat skin
  • Better results on milder, earlier cellulite than on long-established deep dimpling
  • Better results where skin still has reasonable tone
  • Results that build over a course of sessions and continue improving for a period afterwards as collagen forms
  • And results that need maintaining. Cellulite is structural and ongoing. Periodic maintenance sessions hold the improvement; stopping entirely lets it return gradually. We would rather tell you that now than at session six.

We photograph at the start of every course. Gradual change on your own body is genuinely difficult to judge, and the comparison is usually the only honest measure.

Who it suits

  • People close to a stable weight who want the texture improved
  • People willing to commit to a course rather than a single session
  • People who want a realistic improvement rather than a transformation

Who it does not suit

  • Anyone expecting complete removal
  • Anyone currently losing significant weight — better to stabilise first
  • Anyone whose main concern is loose skin rather than dimpling. That is a different treatment conversation
  • During pregnancy and breastfeeding

Cost

Cellulite treatment at Skin Reform starts from R650 per session, with course pricing available. Because the right combination differs by person, we price it at consultation rather than from a menu. Our full price list is published online.

Common questions

Will it come back?

The underlying structure does not change permanently, so yes, gradually, without maintenance. Think of it the way you would think of fitness rather than a repair.

How many sessions?

Typically a course of eight treatments, one to four weeks apart, with results building over the months that follow.

Does it hurt?

Most of these treatments are comfortable. Radiofrequency feels warm, cavitation produces an odd high-pitched sensation in the ear that is harmless and stops when the session does.

Is there downtime?

Generally none. Some redness or warmth in the area for a few hours.

Book a free consultation

We will assess which component is driving what you are seeing, tell you honestly how much improvement is realistic, and show you what a course would involve. Free, at all four clinics, with no obligation at the end of it.
